Use the draw tools to create a small circle. Create you first questions. Copy the circle to how every many answers you want for each question. Then copy the format for your other questions and continue.
I've done this and it works pretty good. Make sure to use the Alignment tools if you need to get the circle to line up with the text.

I don't think there's a template for it...
I would draw circles on the page where you want them to go after you type in the letters.
Ex.
1. A B C D
and then just take the autoshaper and put circles around the letters - don't forget you should be able to copy and paste the letters with the circles to make some duplicates.
