How do I make forms in Microsoft Office Word 2003?

I want to be able to add fields to a form to where an employee can enter their full name in the document. I want the field to be underlined to where the employee will know this is where their name is to be entered. Something like the following ...



Name: ______________________



I want employees to be able to type in text without the underscores moving all over the place. Thanks in advance!

Do not use underscores, draw a line using the line tool in the drawing toolbar. (The drawing toolbar if not showing can be opened by right clicking your toolbars and clicking drawing. Click the line tool and draw a line where you want the underline to be. Hold down shift while dragging your mouse to make the line draw straight. If it will not go where you want it to and snaps above or below where you want it, hold down the alt key while dragging it to where you want it.) You might need to turn off automatically create a drawing canvas when inserting a drawing. Do that by going to the tools menu and clicking options and click the general tab. On there uncheck Automatically create drawing canvas. Once you have drawn the line bring up the forms toolbar by right clicking the toolbars and clicking forms. Put in a text form field by clicking that button (it looks like ab) with the cursor where you want them to put their name. After you finish your document you must protect the document for forms by clicking the Tools menu and clicking protect document and when the pane on the right opens select allow only this type of editing and click the down arrow below it and choose filling in forms. This will only let them type in the text form fields you have created. You can also do other fields too like check boxes and option buttons for your form.
